Your health plan has partnered up with Verdelab and Quest Diagnostics to give you and eligible dependents access to outpatient lab testing at no cost.*
When you have your lab work processed by Quest Diagnostics, you pay no deductibles, no copays, and no coinsurance on covered laboratory services.*
As simple as 1, 2, 3!
Using Quest Diagnostics is optional and if you choose not to use it, your regular benefits will apply. If your doctor’s office doesn’t use Quest Diagnostics, ask them to call 1-800-646-7788 to pick up your test sample.
Find your nearest Quest Lab
Be sure to have your Member ID card with you and tell your doctor to use Quest Diagnostics for our outpatient lab testing.
Tests that are covered
The program covers diagnostic outpatient laboratory
testing, provided the tests have been ordered by your
physician, are covered under the formulary, are approved
by your health benefit plan, and you have remembered
to tell your doctor to use Quest Diagnostics.
- Blood testing (e.g. cholesterol, CBC)
- Cytology and pathology (e.g. pap smears, biopsies) Urine testing (e.g. urinalysis)
- Cultures (e.g. throat culture)
Tests that are NOT covered
- Lab work ordered during hospitalization
- Lab work needed on an emergency (STAT) basis and time sensitive, esoteric outpatient lab testing such as fertility testing, bone marrow studies, and spinal fluid tests
- Lab work performed without the use of your Quest Diagnostics benefit
- Testing that is not approved and/or covered by your current health benefit plan
- Not all lab codes are included in Quest Diagnostics
